Forms

Below you will find some forms that may help you when you are changing your service, moving, transferring a phone number, etc.

• Change of Service Form
  The change of service form should be used by current Xplornet customers who are moving, would like to change the package level of their service, temporarily suspend their service, etc.
• Digital Phone Service LNP Authorization Form
  If you are transferring your existing number to Digital Phone Service, you will need to complete this form. It will permit Xplornet to request the release of your number from your current provider.
• Move Form
  This form should be used by current Xplornet customers who are moving to another location and would like to take their Xplornet service with them.
• Transfer of Ownership Form
  This form should be used by current Xplornet customers who are transferring their Xplornet service and contract to another customer.
• Ottawa Conversion Form
  The change of service form should be used by current Xplornet customers who are living in the Ottawa region and are converting from Satellite (Ka-band or Ku-band) to Fixed Wireless.
